Siekdama aktyvesnio gyventojų dalyvavimo Krūties piktybinio naviko ankstyvosios diagnostikos programoje, Sveikatos apsaugos ministerija (SAM) atnaujino jos organizavimo tvarką. Įgyvendinus bandomąjį projektą, nuo kitų metų pradžios ketinama praplėsti programos dalyvių amžiaus ribas, atsisakyti šeimos gydytojo siuntimo ir įtraukti į programą naują tyrimą – krūties tomosintezę.
Sveikatos apsaugos ministras Arūnas Dulkys teigia, kad prevencinių programų plėtra padės tiek saugant gyventojų sveikatą, tiek didinant visuomenės sąmoningumą, tiek taupant gydymui skirtas lėšas. „Deja, tenka pripažinti, kad prevencijos programos nevykdomos visa apimtimi, dėl to patiriamos didesnės išlaidos gydymui. Kiekviena laiku nediagnozuota liga yra ir papildomas streso šaltinis pacientui. Prevencinių programų pakeitimai padės tiksliau identifikuoti asmenis, kuriems reikia gydymo ankstyvoje stadijoje, valdyti su tuo susijusias rizikas. Prevencinių programų tobulinimas yra nuolatinis procesas, tad gavus bandomųjų projektų rezultatus bus daromi ir kiti pakeitimai tokioje svarbioje srityje kaip onkologinių ligų prevencija“, – teigia ministras.
„Šiuo metu nemokamai atlikti krūties vėžio profilaktiką gali moterys nuo 50 iki 69 metų (imtinai) amžiaus. Tačiau ne visos pacientės žino apie šią galimybę arba turi nepakankamai informacijos apie šios programos svarbą. Dėl to programoje dalyvauja vos pusė tikslinės grupės moterų, nors ekspertai ir tarptautinės organizacijos rekomenduoja ištirti bent 70 proc. rizikos grupės pacienčių. Patobulinta krūties vėžio prevencijos programa ir onkologinių ligų prevencijos programų organizavimo tvarka padės į jas įtraukti daugiau gyventojų, anksčiau diagnozuoti onkologines ligas ir efektyviau jas gydyti“, – sako SAM Asmens sveikatos departamento Specializuotos sveikatos priežiūros skyriaus vedėja Inga Cechanovičienė.
Tobulinama krūties vėžio prevencijos programa
Nuo kitų metų sausio 1 d. visoje Lietuvoje bus praplėstos Krūties piktybinio naviko ankstyvosios diagnostikos programos tikslinės grupės amžiaus ribos – programoje galės dalyvauti 45–74 metų (imtinai) amžiaus moterys. Pacientėms taip pat bus atliekamas naujas šiuolaikiškas tyrimas – krūties tomosintezė, kuri leidžia ištirti net itin mažus (iki 2–3 mm) įtartinus darinius.
Gerinti krūties vėžio prevencijos programos kokybę padeda dešimt naujų mamografų (iš jų keturi – su tomosintezės funkcija), kuriuos Europos Sąjungos (ES) fondų lėšomis įsigijo ir 2020 m. septynioms gydymo įstaigoms perdavė SAM. Vilniaus universiteto ligoninė Santaros klinikos su Nacionaliniu vėžio institutu organizavo mokymus šeimos gydytojams ir radiologams, kaip kokybiškai teikti prevencijos programos paslaugas. Iš viso 2023 m. apmokyta 461 šeimos gydytojas, 121 gydytojas radiologas.
Išbandomas naujas paslaugų organizavimo modelis
Šiuo metu apie krūties vėžio ir kitas prevencijos programas pacientus informuoja ir siuntimus joms išduoda šeimos gydytojai. Nuo kitų metų pradžios šeimos gydytojo siuntimo prevencijos programoms nebereikės, o pacientų informavimą, kvietimą ir paslaugų organizavimą perims du koordinavimo centrai – Vilniaus universiteto ligoninė Santaros klinikos ir Lietuvos sveikatos mokslų universiteto ligoninė Kauno klinikos. Koordinavimo centrų infrastruktūrai paruošti (patalpų remontas, darbui reikalinga įranga ir kt.) buvo skirta 340 tūkst. eurų ES fondų lėšų.
Naujas paslaugų organizavimo modelis bus išbandytas įgyvendinant pilotinį projektą, kuriame dalyvaus abu koordinavimo centrai ir atrinktos gydymo įstaigos.
Vieninga prevencijos programų organizavimo tvarka visoje šalyje leis užtikrinti vienodą programų paslaugų prieinamumą pacientams, nepriklausomai nuo įstaigos, prie kurios jie yra prisirašę, ir padės mažinti sveikatos priežiūros netolygumus.
Jau veikia „žalieji koridoriai“
SAM primena, kad onkologinių ligų ankstyvosios diagnostikos programų metu nustačius piktybinį naviką, paslaugos pacientui organizuojamos per „žaliąjį koridorių“. Šis pacientų, kuriems pirmą kartą įtariama onkologinė liga, srautų valdymo modelis jau taikomas šešiose gydymo įstaigose, kurios sudaro Onkologijos klasterį ir kuriose vykdoma naujo paciento atvejo vadyba. Onkologijos atvejo vadybininkas organizuoja ir registruoja pacientą reikalingoms diagnostinėms ir gydomosioms procedūroms, kol gydytojų specialistų daugiadalykė komanda patvirtins pacientui diagnozę, sudarys gydymo planą ir paskirs pirmąją gydymo procedūrą.
Ministerija primena, kad Lietuvoje vykdomos penkios ligų prevencijos programos, kurių paslaugos apmokamos Privalomojo sveikatos draudimo fondo lėšomis: gimdos kaklelio vėžio, krūties vėžio, prostatos (priešinės liaukos) vėžio, storosios žarnos vėžio ir širdies bei kraujagyslių ligų.
