Kandidatus gali siūlyti pavieniai žmonės, įmonės, įstaigos, organizacijos. Siūlantieji kandidatus gauti valstybės apdovanojimus turi pateikti užpildytą teikimo formą, kandidato asmens tapatybę patvirtinančio dokumento kopiją ir biografiją.
Dokumentus reikia pateikti iki balandžio 2d. Švietimo, mokslo ir sporto ministerijai (A. Volano g. 2, LT-01124 Vilnius) arba atsiųsti el. paštu [email protected].
